The Kindred Transitional Care and Rehabilitation Tunnell Center takes an interdisciplinary approach to care. The dedicated team of caregivers meets with new patients and their families within 24 hours of admission and regularly up until discharge. The team consists of a registered nurse, case manager, social worker, dietitian, activities assistant, physical therapist and/or occupational therapist who continuously review the patient’s plan of care. The goal of everyone at Tunnell is to provide a comfortable and pleasant environment for patients and residents, while consistently providing the highest level of care.
