Acute Care Transition Rounding

Up to a staggering 23% of patients who are discharged from the hospital in the US return within 30 days. Hospitals can minimize readmissions with an effective, seamless transitional care program like rounding.

The Vituity Acute Care Transition (ACT) Rounding Program promotes recovery while minimizing avoidable readmissions through a more efficient, proactive, and compassionate discharge process. Patients receive the resources and support needed to manage their care and recover safely after discharge.

Team Approach for Efficient Patient Care

ACT Rounding allows clinical teams to review the plan of care with the patient and family at the bedside on the evening before discharge. This ensures the patient’s needs are met and provides for a safe and seamless care transition. Patients and families appreciate this high level of communication and support from the entire medical team.

Using the ACT Rounding approach, Vituity’s hospitalist-led multidisciplinary team performs twice-daily rounds to identify and prepare patients for discharge. The team visits lower-acuity patients first, enabling them to be discharged sooner and improving throughput. Team members include physicians, advanced providers, case managers, social workers, nurses, therapists, and pharmacists.

The ACT Rounding framework ensures that discharge planning begins at admission and that communication continues beyond discharge to promote the best possible outcome.

Framework for Daily Rounds

Every afternoon at 2p.m.: The team rounds and identifies patients to be discharged the next day

  • Prescriptions are filled

Evening prior to discharge: Case manager or physician reviews plans with patient and family

  • Patient and family receive education on treatment regimen and next steps
  • Transportation home is confirmed or arranged

Morning of discharge: Reverse rounding

  • Clarify plans if questions arise
  • Discharge before noon to free up beds

Day after discharge: Follow up

  • Contact patient via phone or telehealth-platform to answer any questions
  • Arrange post-acute visits if needed, to reduce readmissions
