Redefining Healthcare Delivery on Patients’ Terms

Health In Place is the unifying concept that the healthcare system should meet patients where, when, and how they need us. As hospitals, health systems, and medical groups established themselves, patient care shifted from the patient’s setting to the medical setting. This was more convenient for healthcare professionals, but not for patients and their loved ones.

Now, Vituity leads a new approach that focuses meeting patients where they need us is enabled by technology, expanded care teams, and a more consumer-driven approach. The upside? This allows us to truly serve as the bridge between patients and their health – tackling challenges that may not be strictly about medicine, but that also account for social services, mental health care, and family dynamics.

As clinicians, we already do this – making sure a homeless patient gets a meal while they’re in the emergency department, having care planning conversations that involve outpatient resources, spending the time to talk through tough diagnoses with family members. But there are so many more ways we can support people’s health in their own context.

By leveraging technology, rural hospitals can engage specialists to conduct patient visits remotely. Likewise, health systems can extend care to non-English speaking patients and those in traditionally underserved communities.
